CVI Form
Please note: Customer dogs from out of state seeking to board with us are required to provide us with a Certificate of Veterinary Inspection (CVI) Form for each dog. The Pennsylvania Department of Agriculture, Bureau of Dog Law Enforcement requires the CVI Form to be completed by an accredited veterinarian from the originating state issued and forwarded to us no more than 60 days prior to the date of entry into Pennsylvania. Following are information and links to forward to your veterinarian to help find the State Animal Health Official who are responsible for creating the CVI Form in each state, and the accredited veterinarians who prepare and certify the CVIs issued in each state.
From Pennsylvania Department of Agriculture, Bureau of Dog Law Enforcement:
What is a CVI? A CVI, also known as a Certificate of Veterinary Inspection is an official document that is: (1) made on a form issued by the chief livestock health official of the state of origin or the United States Department of Agriculture; (2) prepared by an accredited veterinarian of the state of origin certifying the health of the animal described in the certificate; and (3) validated by the chief livestock health official of the state of origin.
